Make Your Will – Step 2

Please fill out the following questionnaire for your Will. Think carefully about your answers. If you are unsure about any of the questions, click the for more information on the topic or refer to our FAQ section.  Remember, the will you make today is not valid until it is signed and witnessed.



When you are satisfied with your answers, click the button at the bottom of the form to see your completed Will.  You can then print the page by choosing File – Print  from the browser menu, or you can copy all the text from the page and paste it into a document of your choice.  When you leave this website, all of your personal information is destroyed to preserve your privacy.

After viewing and printing your Will, you can go to the Documents section to print a Cover Page for your Will, as well as a Documents Locator and a Funeral Preferences page.
